University Healthcare Berkeley Medical Center has been recognized by Premier, Inc. for reducing preventable hospital-acquired conditions (HACs) and re-admissions, as part of the federal Partnership for Patients initiative.
Through the Partnership for Patients, Premier’s hospital engagement network (HEN) is working with more than 400 hospitals to reduce preventable re-admissions by 20 percent and preventable HACs by 40 percent from January 2012 – December 2014.
As a member of Premier’s HEN, Berkeley Medical Center is one of the 28 hospitals to be honored as a Premier Partnership for Patients Award finalist for early success in reaching its reduction goals in at least six of the 10 areas measured. Berkeley Medical Center was recognized during Premier’s annual Breakthroughs Conference and Exhibition in San Antonio.

Specifically, Berkeley Medical Center has met and/or exceeded the Partnership for Patients reduction goals in the following areas: greater than 40% reduction in HACs and greater than 20% reduction in re-admissions.

Premier is the second largest HEN in the Partnership for Patients initiative working at the regional, state, national and hospital system level to identify solutions to reduce HACs and re-admissions, and spread best practices and resources to other healthcare providers. Participants collaborate to:
- Transparently share progress on a set of uniform measures on a regular basis;
- Share best practices and successful innovations with all members of the HEN; and
- Commit to participate in educational initiatives and site visits.
